Roselands Doctors are starting a regular update to let our patients know about our Continuous Quality Improvement projects.
The aim of these is to improve the healthcare for all our patients.
Our current projects are for 2025
- Improving our Cardiovascular Risk Assessment targets.
You may have received texts or emails from us asking you to do bloods tests for cardiovascular risk The blood tests include your cholesterol, sugar levels and kidney function. We can plug this data plus you recent blood pressure and other information into a formula which can identify your risk of heart attack or stroke over the next 5 years. While not perfect, it is a good start. If you score in the high risk category, you will get a call from us to come and talk to your doctors about managing your risk. This may include lifestyle advice, support with diet and weight loss , medications for cholesterol of recommendations for more investigations with specialists. The aim of all this is to prevent heart attacks and strokes and keep our patients healthy!
Our project ,which started in January 24 has shown we have improved our practice population assessment from 68 % to 88%. Our target is 90 % so we are very happy with this improvement.
This has been a team effort with all our nurses, doctors and clinical pharmacist Agnella all pitching in.
2) Ensuring all our patients with Atrial Fibrillation have been assessed by a score called the CHADSVAC which can identified patients at risk of stroke who need to be on blood thinners.
Dr Steve has run this project and we have improved fro 53 % of AF patients being assessed to 97.5%.
3) We are also working on our other cardiovascular management targets, smear recalls (please come in for your smear!) and childhood immunisations.
4) We will shortly be starting a nurse led FREE diabetes clinic for all our diabetic patients. Our nurses are trained and will be supported by our pharmacist and the doctors. This means that your diabetes management should improve and you will not need to see your doctor so often.
